Home Blood Pressure Monitoring

If you have been asked to monitor your blood pressure at home by the Practice, you can click here to download the form. This form can be filled in electronically and emailed back to us at the end of the monitoring.

Blood pressure should be recorded twice a day for at least 7 days. Blood pressure is taken as 2 consecutive measurements at least 1 minute apart, both morning and evening. You should be relaxed in a quiet, comfortable environment at normal room temperature and should not be talking. Ideally you shouldnt need to pass urine, and should not have eaten recently, smoked, taken caffeine or exercised. You should be seated for at least 5 minutes and any tight clothing should be removed before taking the reading. Your arm should be supported at heart level.

Blood pressure forms must have your name and date of birth on them and must be fully complete, or you may be asked to redo the monitoring.

Any blood pressure monitors borrowed from the Practice, must be returned at the end of the 7 day period.

Forms that have been filled in electronically must be emailed to nl.b81118@nhs.net at the end of monitoring.
